Please note that both the pdf and word file applications MUST be signed before posting to the Agency.

Please note photocopied / faxed application forms will not be accepted.

Please note that all completed application forms should be sent to the Ulster-Scots Agency at least eight weeks before the date of event in order for it to be fully considered.
